Code of Conduct - Statement of Acknowledgment and Agreement I have received and read a copy of the Creekside Community Church - Children's Ministry/Student Ministries Code of Conduct Policies and Procedures and understand the importance of the matters set forth within the material. I agree to observe and conduct myself in accordance with these guidelines during my service at Creekside Community Church. I hereby acknowledge receipt of the Code of Conduct.
